What Is the USMLE?
The USMLE stands for the United States Medical Licensing Examination. It’s a three-step exam series required to practice medicine in the United States. International medical graduates—including those from India—must pass these to obtain a U.S. medical license.
Exam Structure & Pattern (Step by Step)
Step 1
- Format: Computer-based, one-day test.
- Duration & Blocks:
- 8 hours total
- Divided into 7 blocks, each 60 minutes long
- Up to 40 MCQs per block → maximum 280 questions total
- Breaks & Tutorial:
- 45 minutes of break time
- Optional 15-minute tutorial at the start

- Questions: 316 MCQs across 8 blocks
- Format: Two-day exam
- Day 1: ~233 MCQs over 8 hours

What Topics Does Each Exam Cover?
Step 1 focuses on basic medical science and its application, covering:
- Anatomy, Biochemistry, Physiology, Pathology, Pharmacology, Microbiology
- Behavioural sciences, Genetics, Immunology, Nutrition
- Epidemiology, Medical Ethics, Biostatistics, Cellular Biology, Patient Safety.
Step 3 assesses your decision-making in real-world medical scenarios, including simulations. (Syllabus detail wasn't found but generally follows clinical reasoning.)
Fees for Indian (international) candidates include both the base exam charge and a surcharge for testing outside the USA/Canada.

Eligibility & Application Process
- Step 1 & Step 2 CK: You must be a graduate or student at a medical school listed in the World Directory of Medical Schools and meet ECFMG eligibility criteria.
- Step 3: You must have passed Step 1 and Step 2 CK and hold an MD (or equivalent), plus ECFMG certification.
- How to apply:
- Register via ECFMG’s IWA system for Steps 1 and 2, FSMB for Step 3.
- Obtain your USMLE/ECFMG ID, schedule a 3-month eligibility period, pay fees, and book via Prometric centers.
